The grouping properties should be abbreviated in the same way, no matter what table they are in. Here are some differences that I noticed when I wrote the document “How to read NordDRG definition tables 2021-12-20".

The uniformity should be achieved in the easiest way, which can be decided by NCC.
Furthermore, it has been pointed out that the difference between ‘procpro’ and ‘secpro’ in the table drg_logic is unclear for those who are not experts in the grouping logic. The suggestion from our IT consultant is that these headings are changed to ‘procpro1’ and ‘procpro2’ similar to the ‘dgprop’ columns.
All suggested changes in this case can be regarded as cosmetic, but they increase the transparency of the system, which is good for newcomers, both in local and national DRG teams. There is no hurry, however. The changes can be done when appropriate, but at latest when NordDRG shall be adapted to ICD-11, when the groupers must be re-programmed anyhow.

It was decided during EG meeting that the changes will be done in 2024 PL version, change concerns the NDMS, grouper (Nordic and national) and all def. tables (xls, json, csv).

Given that the introduction of ICD-11 and ICHI will require changes in NDMS, grouper and def.tables, this change will be done once the actions concerning the transition to ICD-11 and ICHI will be taken.

This is to clarify the issues which require changes in NordDRG tools (NDMS, grouper, def. tables). Based on that, the original text of this ticket was changed from "DRG-MDC vs DG-MDC" to “Changes required in NordDRG tools (NDMS, grouper, def. tables)” to reflect the actual content of this ticket.
The issues are taken from a Word file attached to #822.
They are not directly related to transition to ICD-11 but given that the transition will require changes in NDMS, grouper and/or def.tables anyway, it would make sense to manage all the necessary changes at once.
The suggested changes are as follows:
- Change the heading ‘mdc’ in the table drglogic to ‘mdc_dg’ (= mdc of diagnosis) and the heading ‘mdc’ in the table drg_name to ‘mdc_drg (= mdc of drg)’ This will show that the term MDC is used in two different contexts in NordDRG
- In the table drg_logic, rename the columns procpro and secpro to procpro1 and procpro2, similar to the dgprop columns. The term “secpro” is not present in the table proc_feat and the meaning is not obvious.

Principal diagnosis property
- Extra columns in the table drglogic for administrative parameters as the type of care, type of contact and type of profession of the health care personal. Currently they are in the fields for diagnoses and procedures.
- Extra columns in the table drglogic for interventions. Now there are only two columns for interventions so the columns dgprop 1-4, originally meant for diagnoses, have to be used also for interventions if a grouping rule is based on more than two procedure codes.
- Enable multiple coding for procedures (similar to asterisk-dagger for diagnoses) so that general qualifiers (NCSP codes beginning with Z) are hooked to the right intervention code. With the present logic, an irrelevant bilateral procedure (e.g. peripheral intravenous catheters) can lead to a DRG for major bilateral interventions (e.g. DRG 471N ‘Bilateral or multiple major joint procs of extremities’). If the problem will be solved in ICHI, we can skip this suggestion.

As we are making changes in NordDRG tools, couldn't we create a separate table for ATC codes at the same time? A separate table would work in the same way as dg and proc today, a parent table. SWE currently has no ATC codes that affect the grouping, but we have a lot of ATC codes. In SWE, the drug must be registered in case of poisoning and side effects.

We have had discussions about the maintenance of ATC codes on Forum tickets #690 and #853.
In NDMS/MBC we have separate tables for ATC codes (national and ATC+) available already.
We also updated the template of TCs in 2023 and added a separate sheet for ATC codes (see the file ‘TC_TEMPLATE_2021-12-08_updated.xlsx’ under the Files tab in Forum https://nordcaseforum.easyredmine.com/projects/ncc/files?utm_campaign=menu&utm_term=files
Given the different use of ATC codes in national definition tables, the full potential for managing and maintaining the ATC codes in NDMS/MBC is still to be realized.
